As a new client, You will be asked to share some basic profile information, details about your current concerns, your medical and family history, and a few questions about your health beliefs. This helps us better understand your needs and guide your care. You will also be asked to review and sign several important documents, including the Consent to Treat, HIPAA Privacy Policy, Cancellation Policy, and Out-of-Pocket Payment Agreement. Once your appointment is scheduled, you’ll get an email with a link to complete it online. Prefer paper? Arrive 15 minutes early to fill it out in the office.
Your first visit typically includes an initial evaluation followed by an intervention session.
Fee details are outlined on the intake form, and we encourage you to reach out with any
questions beforehand to ensure you're comfortable with the fee structure. For your initial
appointment, please wear or bring loose, comfortable clothing such as leggings, sweatpants,
or shorts.
